What is an Annuity Certificate?

An Annuity Certificate is the official proof of the retirement benefit now being provided by Pacific Life. 

Why am I receiving an Annuity Certificate?

You are receiving an Annuity Certificate because your retirement benefit is now provided by Pacific Life under a group annuity contract. 

What is a group annuity contract?

A group annuity contract is issued by an insurance company to provide benefit payments to individuals of a pension plan. When a pension plan purchases a group annuity contract, the insurance company then becomes responsible for the payments specified in the contract. 

How and when will I receive my Annuity Certificate?

The Annuity Certificate will be mailed to you after Pacific Life has received all necessary plan and regulatory approvals.

What happens if I lose my Annuity Certificate? 

If you lose your Annuity Certificate, please log into your Pacific Life Member Self Service account or call Pacific Life Member Services at (833) 702-1617.  

What information is included in my Annuity Certificate?

For individuals who have not yet retired:
Your certificate includes your certificate number, the date on which you can commence full benefit payments, the estimated annuity amount you will receive, and the annuity options available to you. 

For annuitants who are retired:
Your certificate includes your certificate number, a description of your annuity benefit, your payment amount, the date payments begin with Pacific Life, your Joint Annuitant (if applicable), and the Beneficiary you have elected (if applicable). 

I was not a participant in the pension plan. Why did I receive an Annuity Certificate?

If you were not a plan participant, you likely received an Annuity Certificate for one of the following reasons:

1. You are a joint annuitant or beneficiary of a deceased plan participant.

2. You are an Alternate Payee of a plan participant under a Qualified Domestic Relations Order.

The personal information on my Annuity Certificate is incorrect. What should I do?

If there is any incorrect information on your Annuity Certificate, please call Pacific Life Member Services as soon as possible at (833) 702-1617. 

What is the Annuity Certificate number and why do I need it?

The Annuity Certificate number is the unique identifier assigned specifically to your annuity contract. It may be found on your Annuity Certificate. Please refer to this number when communicating with us to help us locate your records.

To protect your privacy, Pacific Life does not use your Social Security number (SSN) in any correspondence. 

What is a Joint Annuitant?

A Joint Annuitant is the person entitled to receive payments after the Annuitant’s death.

Can I change my Joint Annuitant to another person?

No. The election of a Joint Annuitant and Survivor Annuity and person designated as the Joint Annuitant are irrevocable and therefore cannot be changed. 

What is a Beneficiary?

The Beneficiary is the person you name who may receive the death benefit proceeds or any remaining payments if you or your Joint Annuitant (if applicable) passes away. 

How do I add or change my Beneficiary?

Beneficiary elections and changes are only accepted in writing. Please call Member Services at (833) 702-1617 to receive a Pacific Life Policy Change Request Form or download it here. Once you have downloaded the form, return it to Pacific Life either via U.S. mail, fax, or by uploading it to your Pacific Life Member Self Service account.

What are Period Certain payments?

The number of Period Certain payments shown on your Annuity Certificate is the total number of annuity payments that are to be paid to you or your Beneficiary regardless of when you may pass away.

Why doesn’t the number of Period Certain payments on my Annuity Certificate match the number of payments I selected on my original retirement date?

The Annuity Certificate reflects only the remaining Period Certain payments that Pacific Life is responsible for. Any payments made directly by the Plan before the annuity commencement date are not included.
