How do I know if a provider is in-network?

To find an eye care provider within our network or see if your current provider is included, you can access our convenient provider search link. Don’t see your eye doctor listed? You can  nominate your provider  for network membership.

How do I access my online portal account?

Your online Pacific Life Portal gives you the ability to manage all your benefits in one convenient location. After you enroll, you’ll receive an email invitation with a link to create your account. If you need support, call us at (855) 810-3301. Once your account is set up, you can sign in.

How do I submit a claim?

When you visit an in-network provider, you do not have to submit a claim. If you do choose to go out-of-network and your plan has out-of-network benefits, you will need to pay during the visit and then submit a claim for reimbursement. To access the out-of-network claim form, sign in to your portal.

Do discounts exist for buying glasses and contacts in the same year?

If you visit an in-network provider, you may receive discounts including 40% off an additional complete pair of prescription eyeglasses and 15% off additional conventional contact lenses after your benefit has been used, plus more. Additionally, sign in to your portal and receive special offers from retailers including LensCrafters® and Target Optical® that are updated quarterly.

What will my glasses or contacts cost?

You can easily receive an estimate of your total costs ahead of time with the out-of-pocket cost estimator available in your portal. The tool incorporates your specific plan benefits to calculate your savings and estimate your balance, so there are fewer surprises when it is time to pay your provider.
